My setup is:

Instrument or Microphone > Preamp (Vintech Dual 72, FMR RNP, or M-Audio Octane) > FMR RNC (bypassed most of the time) > M-Audio 2496 Soundcard > Sonar 8

OR (when recording drums):

Instrument or Microphone > Preamp (Vintech Dual 72, FMR RNP, or M-Audio Octane) > FMR RNC (bypassed most of the time) > AW16 G ----- export tracks to disc and import to Sonar 8 for mixing.

AND sometimes:

Musical idea comes out of my ass> Instrument or Microphone > Preamp (Vintech Dual 72, FMR RNP, or M-Audio Octane) > FMR RNC (bypassed most of the time) > M-Audio 2496 Soundcard > Sonar 8.

Okay, when doing a live recording:

Instruments -> board -> out of board inserts -> 1600 !

I never know until the day of when I'm going somewhere, so you'll get a last minute call and you'll be busy and not be able to make it...story of my life!
